When Should You Go to The ER with a Hemiplegic Migraine Attack?

Updated: Aug 11, 2022

This is an important question to know the answer to. When we have a hemiplegic migraine attack, our symptoms are so severe because we have symptoms which mimic a stroke. We lose the ability to speak, the use of our arms and legs on one side, dizziness, nausea and vomiting. Many times, the ER doctors believe we are actually having a stroke. But if we went to the ER every time we had an attack, we would be there quite often. I asked a headache specialist when they thought we should head to the ER. This is what they advised.

WHEN YOU SHOULD HEAD TO THE ER:

  • If you cannot manage your symptoms at home.

  • If you are vomiting for over 24 hours.

  • If your stroke-like symptoms come on SUDDENLY...you should be evaluated immediately for stroke.

  • If you are having new symptoms which are concerning you.

It also a good idea to have your neurologist write a protocol for the ER doctors to follow so they know how to treat your hemiplegic migraine. Many times I have had to explain to the ER doctors how to treat my HM while having an attack!!!!

Having to go to the ER is a difficult decision to make. Following these guidelines and also having a protocol for ER doctors can help us during these challenging times.
